FHS REQUIRED FORMS (checklist): These must be completed before an athlete can participate in any way. Please have them ready for SATURDAY, AUG. 28, 2010.

Sports Physical
NOTE: The physical must have been completed within one year of participation. If your physical expires during the season you must have it renewed or you can’t participate until it is done. Call the school office and ask for the Nurse if you’re not sure. 673-2514.
